Multiplate®HirudinBloodTube(Double-Wall)isabloodcollectiontubeforuseinplatelet aggregationstudieswiththeMultiplate®plateletfunction analyzertoevaluatequalitativeplateletdisordersat physiologicalcalciumconditions. Hirudin,athrombininhibitorallowsanticoagulationof bloodwithoutinterferencewithphysiologicalcalcium levels.

DuringtheearlyphaseofMultiplate®development,citratedblood(either3.2%or3.8%)was usedforthesamplecollection. Itwasobservedthatsubjectsdidnotaggregateproperly,particularlywithADPasagoNIST.Citratecomplexescalciumandtherebypreventsclottingofthesample.However,calciumisalsoanimportantsecondmessengerforplateletaggregation.Forafullshapechangeandfullexposureofreceptorsontheplateletsurface,thereleaseofcalciumfromtheintracellularstoresisnecessary.Thebestsolutionforthisproblemistouseananticoagulantwhichdoesnotaffectthefreecalciumconcentrationinthesample, ideallyhirudin(adirectthrombininhibitor).Heparinhassomeactivatingpropertiesonplatelets;therefore,theuseofhirudinbloodispreferred.Toreducetheinfluenceofcalciumdepletionbycitrate,apartialrecalcificationusingtheNaCl/CaCl2diluentsolution isused.ThepartialrecalcificationcanleadtoadisaggregationphenomenonintheASPItestandRISTOtest.Forthesetests,0.9% salineisusedwhentesting citratedblood.
