Cortisol(alsoknownashydrocortisone,compoundF)isthemainglucocorticoidinhumansandisproducedinthezonafasciculataoftheadrenalcortex.90%ofthecirculatingcortisolareboundtocorticoidbindingglobulin(CBG,Transcortin),ca.7%areboundtoalbuminandonly1–3%areunbound.Onlythelatterpartrepresentstheactiveformofcortisol.Thefreecortisolisreleasedinsalivaandisexcretedviathekidneysasasmallpartamongthemetabolitesofcortisol.TheleveloffreecortisolinbloodregulatesmainlyitssecretionintheadrenalcortexinanegativefeedbackmechanismviaCRH(corticotropinreleasinghormone)inthehypothalamicregionandtheACTHinthepituitarygland,butitisalsoaffectedbydifferentsituationsaboveallbystress.

Inhumansthereisaphysiologicalfluctuationofcortisolachievingthehighestlevelinthemorningandthelowestduringmidnight.Thisfluctuationofcortisolplasmalevelisreflectedinsalivanormallywithapeakinthefirst90minutesafterwakeup.Thecortisolmeasurementisindicatedinadrenaldisorders.Duetothediurnalfluctuationsofcortisolasalivarysamplecollectionisaneasymethodwithoutthestressofrepeatedvenipunctures.
